EVERYONE MUST REGISTER INDIVIDUALLY. WE NEED INFORMATION AND A RELEASE FORM FOR EACH PERSON SERVING.
ONE PERSON PER REGISTRATION ONLY.

**This registration is for those serving at the night of the event only** A separate registration form is provided for those serving in "Pre-Event" opportunities.  

** You may register for ONE opportunity to serve AT the event.  Do not register for more than ONE opportunity.**

**Minimum age for all Life Changers is 16 years of age**
